Story Framework: Triaging a Post-Launch AI Failure and Rebuilding Trust

Situation: You're the engineer responsible for a RAG-based knowledge assistant serving internal users or external clients. After launch, users start reporting that "the assistant's answers are off-topic" and occasionally cite completely irrelevant policy clauses. Every AI Engineer who has shipped a RAG system will hit this eventually — and it's a favorite scenario for interviewers testing how you react when things break.

Task: You're expected to identify the root cause quickly without taking the service offline, while also reassuring stakeholders (product managers, client contacts, or front-line users) who have already lost confidence in the system.

Action: Start by explaining how you narrowed down the problem — not by rushing to tweak the prompt or swap models, but by first determining whether the issue was in retrieval or generation. Specifically: you sampled cases and laid out "user query → retrieved chunks → model's final answer" side by side, and found that in most cases the wrong answer wasn't the model's fault — the retrieval stage was already feeding it the wrong context. Then explain how you dug deeper: for example, you discovered that chunks were split at a fixed 512-token boundary with no regard for sentence boundaries, causing cross-boundary content to be cut mid-sentence. With broken semantics, the model could only do its best with incomplete context. After switching to sentence-boundary-aware chunking, answer quality stabilized noticeably. Finally, explain how you rebuilt trust: not by quietly pushing a fix and moving on, but by proactively building a representative golden test set (e.g., 80 questions covering various edge cases) and showing stakeholders quantified before-and-after accuracy — so what they saw wasn't "I fixed it" but "here's how I know this won't happen again."

Result: Quantify the improvement (e.g., "retrieval hit rate went from X% to Y%," "customer complaints dropped by Z%"), and explain what mechanisms you put in place to catch similar issues faster in the future (e.g., running the golden test set on a schedule, adding retrieval quality monitoring).

How to Tell This Story

  • Do: Lead with how you defined the problem before jumping to the solution — interviewers care about whether you separated retrieval from generation diagnostics. This is the biggest differentiator between senior and junior engineers.
  • Do: Volunteer a line like "I initially thought it was A, but it turned out to be B." This kind of honest self-correction is more convincing than a story where everything goes perfectly.
  • Do: Frame "rebuilding trust" as concrete mechanisms (test sets, monitoring, process) rather than vague "better communication."
  • Don't: Blame the model or third-party tools. The interviewer is listening for how you take ownership, not how you deflect.
  • Don't: Skip quantified results — without numbers, interviewers can't gauge the actual scope of your impact.
